To submit a claim for cash reimbursement, do the following:
Complete, sign, and date a Claim Form, and mail the Claim Form and your supporting documentation no later than October 1, 2016 to the following address. (Keep a copy of the completed Claim Form).
To exclude yourself from the Settlement, you must fully complete and sign the Request for Exclusion Form and return by U.S. mail (or an express mail carrier) postmarked no later than June 13, 2016 to:
If you timely submit a fully completed and signed Request for Exclusion Form by U.S. mail or express mail, you will not be able to receive any benefits of the Settlement and you cannot object to the Settlement. You will not be legally bound by anything that happens in this lawsuit.
